1. Parties

Contractor: Wolff Development LLC, 4340 E Indian School Rd #21-537, Phoenix, AZ 85018. Arizona Registrar of Contractors License No. 328444 (KB-1 Dual Building Contractor). Phone 520-686-1483. Email zacharyjayers@gmail.com.

Homeowner: enter your name, contact details, and the property this Agreement covers.

2. Insurance Claim Information

Enter your claim details so we can coordinate with your insurer.

Adjuster information (optional — if available)

3. Inspection Disclosure

The property was inspected before this estimate was prepared, and the roof was physically accessed during the inspection. The Contractor will provide the Homeowner with a precise description and location of all damage claimed, supporting photographs or digital images, an itemization of any emergency repairs already completed by the Contractor, and a specification of any damaged areas not included in the estimate and the reason for their exclusion.

4. Scope of Work

Contractor will repair or replace the roof system at the property as described in the insurance carrier's approved scope of loss and any approved supplements, including removal and disposal of existing roofing, underlayment, flashing, ventilation and new roofing materials of equal or better quality as specified in the approved scope. Work outside the approved insurance scope is performed only under a written change order signed by the Homeowner.

5. Contract Price

The Contract Price is the amount approved by the Homeowner's insurance carrier for the approved scope of loss, including any approved supplements (ACV plus recoverable depreciation) — that approved amount is our estimate for the work. Any additional upgrades or repairs requested by the Homeowner that are not covered by the insurance scope will be priced and agreed in a signed change order by both parties before that work is performed.

6. Payment Terms

(a) The insurance carrier's ACV (actual cash value) payment is due to Contractor at the start of the project. (b) The remaining RCV (replacement cost value / recoverable depreciation) balance is due upon completion of the installation, once photos and completion documentation have been submitted to the carrier, and is payable to Contractor when released by the carrier. (c) DEDUCTIBLE: It is the Homeowner's responsibility to pay their insurance deductible. However, the Homeowner's out-of-pocket expense will not exceed the deductible amount unless the Homeowner requests additional upgrades or repairs that are not covered by the insurance scope. (d) With the Homeowner's written consent, the insurer may issue its check in the names of both the Homeowner and the Contractor, with Contractor's license number noted on the check.

7. Insurance Disclosures

Contractor has made no assurances that the claimed loss will be covered by an insurance policy. The Homeowner remains responsible for their insurance claim. Contractor does not act on behalf of the Homeowner in negotiating the settlement of a claim; with the Homeowner's permission, and without compensation for that communication, Contractor may communicate with the insurer to assist with claim disputes, including actual damages incurred. The policyholder is responsible for payment for any work performed if the insurer should deny payment or coverage for any part of the loss.

8. Right to Cancel

You may cancel this contract at any time within seventy-two hours after you have been notified that your insurer has denied your claim to pay for the goods and services to be provided under this contract. You may cancel this contract at any time, for any reason, within four business days after you sign this contract.

9. Governing Law & Dispute Resolution

This Agreement is governed by the laws of the State of Arizona. Any dispute arising from or relating to this Agreement shall first be submitted to good-faith mediation. If mediation does not resolve the dispute within thirty (30) days, the parties agree to binding arbitration in Maricopa County, Arizona, under the Arizona Revised Statutes. The prevailing party is entitled to reasonable attorney fees and costs.

10. Warranty

Contractor warrants its workmanship for two (2) years from the date of substantial completion against defects in installation. Manufacturer material warranties (lifetime) pass through to the Homeowner subject to the manufacturer's terms and conditions. This warranty does not cover damage from subsequent storms, improper maintenance, or modifications made by others. Warranty claims must be submitted in writing within the warranty period.

11. Permits & Code Compliance

Contractor shall obtain all required building permits and perform the work in compliance with applicable Arizona building codes and local municipal requirements. Permit costs are included in the Contract Price unless otherwise noted in a signed change order.

12. Severability & Entire Agreement

If any provision of this Agreement is found to be unenforceable, the remaining provisions shall remain in full force and effect. This Agreement, together with the approved scope of loss and any signed change orders, constitutes the entire agreement between the parties and supersedes all prior negotiations and representations.

13. Solar Detach & Reset

If the property has solar panels, a detach & reset may be required to access the roof surface. Select whether the property has solar requiring a detach & reset so we can coordinate it with your approved scope.

14. Additional Notes

Add anything else we should know about the property, the damage, or the approved scope.

15. Homeowner Signature

By signing, you agree to hire Wolff Development LLC to perform the insurance-approved scope of work described above. Work outside the approved scope requires a signed change order. This Agreement is contingent on insurance approval.

16. Co-Signer / Joint Owner

For community-property homes, add a joint owner so both owners can sign this Agreement. Optional — leave unchecked if the property has a single owner.

17. Home Improvement Specialist

Completed and signed by the Wolff Development home improvement specialist responsible for this agreement.
